Sai Pallavi was born into a Hindu household in Kotagiri, Nilgiri, Tamil Nadu on May 9, 1992. Santamaria Kannan, her father, is a central excise officer, and Radha Kannan, her mother, is a housewife. She is one of two brothers and sisters. Puja Kannan, her younger sister, is also an actor.

Sai attended Avila Convent School in Coimbatore, Tamil Nadu, for her higher education. She then went on to Tbilisi State Medical University in Tbilisi, Georgia, where she earned her MBBS. She received her medical degree in 2016, however, she is not a licensed physician.
